Career Shift is committed to providing quality services and ensuring our clients are comfortable in entrusting their personal information. This policy describes how we handle your personal information.
We take our obligations under the Privacy Act 1988 (Cth) (te Privacy Act) seriously and take all reasonable steps in order to comply with the Australian Privacy Principles contained in the Privacy Act. A copy of the Australian Privacy Principles may be obtained from the website of The Office of the Australian Information Commissioner at www.aoic.gov.au
Why and When Your Consent Is Necessary
Career Shift will need to collect and record personal and/or sensitive information related to our client’s current situation and the scope of services to be provided.
When you are referred to Career Shift as a client, you provide consent for our Rehabilitation Consultants, Vocational Consultants and practice staff to access and use your personal information so they can provide you with the best possible rehabilitation services. If we need to use your information for anything else, we will seek additional consent from you to do this.
We will request your consent in writing during your Initial Rehabilitation Assessment and annually after that.
If you do not provide some or all of the personal information we request, we may be unable to effectively provide our services to you. The referring party will be notified (if the services were not self-referred) to discuss the implications on services as a result of consent being declined.
When we collect Personal Information, we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.
What Is Personal and Sensitive Information?
Personal information is information or an opinion that identifies an individual. Examples of personal information we collect include; name, address, date of birth, email address, phone and facsimile number.
Sensitive information is also personal information and includes information or an opinion about your:
- Racial or ethnic origin
- Political opinions
- Religious beliefs or affilitations
- Philosophical beliefs
- Sexual orientation
- Criminal record
- Health information
- Genetic information.
Personal Information Use and Disclosure
We may collect personal and/or sensitive information from individuals that is reasonably necessary for, or directly related to, the services we deliver (such as rehabilitation services and vocational services), or if required by law.
This may include health information as well as information about your circumstances, such as your employment status and history, education status, financial situation, cultural and linguistic background, welfare, disabilities, and family circumstances.
The types of information we collect from you depends on the purpose for which we require the information.
Career Shift collects personal information to enable us to conduct business within our scope of services, including:
- Determining an individual’s requirements for appropriate services.
- Setting up and administering services.
- Tailor services to clients’ needs.
- Evaluate and monitor outcomes, programs and services provided.
- Meeting contracted obligations held with our customers and related stakeholders.
Personal information may be used for purposes other than for which it was collected, namely,
- To prevent a serious threat to a person’s health or life.
- As required or authorised by law.
- Where reasonably necessary for the enforcement of criminal or revenue law.
- Where summoned, subpoenaed or where a freedom of information request is received by an authorised person or the client and compilies with the Privacy Act’s National Privacy Principles and our contractual obligations.
How We Collect Your Personal Information
We will, where possible, collect your personal information directly from you, unless it is unreasonable or impractical for us to do so. If we collect your personal information from another person and it is unlcear that you have consented to the disclosure of that information to us or that information is otherwise not permitted to be disclosed to us, we will whenever reasonably possible, make you aware that we have done this and the reasons for doing so.
For example, we may collect personal information from you through telephone calls, SMS, your emails, website contact forms, social media and other correspondence to us, as well as in person.
In some circumstances personal information may also be collected from other sources. Often this is because it is not practical or reasonable to collect it from you directly. This may include information from:
- Your guardian or responsible person.
- Other involved healthcare providers, such as specialists, allied health professionals, hospitals, community health services and pathology and diagnostic imaging services.
- Referring organisations, insurance companies, WorkCover QLD or Department of Veterans’ Affairs (as necessary).
In such circumstances we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.
Dealing with us Anonymously
You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals.
How We Store Personal Information
We will take all reasonable steps to protect your personal information against misuse, interference and loss, and from unauthorised access, modification or disclosure.
These steps include:
- Storing paper records securely either at our office or at an off-site secure storage facility;
- Storing electronic records in a controlled and secure environment;
- Only accessing personal information on a need-to-know basis and by authorised personnel for the purposes of carrying out their work on our behalf;
- Monitoring systems access which can only be accessed by authenticated credentials;
- Ensuring our buildings are secure; and
- Regularly updating and auditing our storage and data security systems.
When no longer required for any of our business functions, or for any other lawful purpose, we destroy or de-identify personal information in a secure manner.
Other than in the course of providing our services or as otherwise described in this policy, Career Shift will not share personal information with any third party without your consent. We will not share your personal information with anyone outside Australia without your consent (unless under exceptional circumstances that are permitted by law).
Access To Your Personal Information
Except in exceptional cases established by law, you may request access to the personal information we hold about you by writing to our Privacy Officer at the address below. You do not have to provide a reason for requesting access. We will aim to respond within 30 days of your written request being received. We may require proof of identification from you to release the information.
If we hold personal information you are entitled to access, we will endeavour to provide you with a suitable range of choices as to how you may access that information (e.g. post or collection).
If you believe that personal information we hold about you is incorrect, incomplete or inaccurate, you can ask us to amend it. We will consider your request and:
- If we agree that the information we hold is inaccurate, we will amend it; or
- If we do not agree, then we will add a note to the personal information stating you disagree with its accuracy.
Updating Your Personal Information
It’s important to tell us if your personal information changes. We will take reasonable steps to make sure that your personal information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practical so we can update our records and ensure we can continue to provide quality services to you.
Website Usage Information and Cookies
Cookies: Our website uses small data files called cookies on your computer, which you can choose to accept or decline. Cookies are very small files which a website uses to identify you when you come back to the site and to store details about your use of the site. Cookies are not malicious programs that access or damage your computer. Most web browsers automatically accept cookies but you can choose to reject cookies by changing your browser settings. However, this may prevent you from taking full advantage of our website.
Website: There is also information about the hardware and software on your computer that is automatically collected by our website. This information can include your IP address, browser type, domain names, browsing preference, access times and the addresses of referring website. This information is used by us to maintain the quality of our website and to provide us with information regarding the use of our website.
Third Party Sites: Our website may from time to time have links to other websites not owned or controlled by us. We encourage you to review the privacy statements of websites you choose to click through to from our website so that you understand how those websites collect, use and share your information. We do not have control over and are not responsible for the manner in which the hosts of other websites use personal information they collect from you.
Policy Updates
This Policy will be reviewed on a regular basis and Career Shift reserves the right to change any of the provisions set out in this document from time to time, as may be required.
